Authorized staff members can securely log into the Secure Broker Site to update member information, such as name, address, and primary care physician. How can my client enroll and change employee records electronically?
You or your clients may make changes to an enrollee’s records (such as name, address, and primary care physician) via the Secure Broker Site. This user-friendly option enables CDPHP to receive changes quickly and securely. Learn more.
What paperwork needs to be submitted to enroll a new small group with CDPHP?
You will need the following documents:
- Completed employer application.
- Proof of business eligibility (New York State 45 form).
- Broker of record letter on client’s letterhead (if group was sold by a broker).
- Check for first month’s premium.
- Completed member applications.

Use our Find-A-Doc tool to locate a doctor in our network or to see if your doctor is already a part of CDPHP. The Contributory PPO and Voluntary PPO dental plans are administered by APA Partners, Inc., and utilize the APA Partners dental network.
