You or your clients may make changes to an enrollee’s records (such as name, address, and primary care physician) via the Secure Broker Site. This user-friendly option enables CDPHP to receive changes quickly and securely. Learn more.

What paperwork needs to be submitted to enroll a new small group with CDPHP?

You will need the following documents:

  • Completed employer application.
  • Proof of business eligibility (New York State 45 form).
  • Broker of record letter on client’s letterhead (if group was sold by a broker).
  • Check for first month’s premium.
  • Completed member applications.
