Submitting a Request

Sponsorship requests can be submitted by mail, fax, or email to:

Guidelines for Sponsorship Requests

Submit your request at least 60 days prior to the planned event or program.
Provide a detailed written description of your event or program, including: date, time, place, level of support requested ($), and contact information.
Describe the purpose of the event or program and the potential benefits CDPHP might derive from the sponsorship. Include information on expected attendance, potential marketing and promotional opportunities, and how the program or event might relate to the mission of CDPHP.

Note: CDPHP does not sponsor annual appeals, capital campaigns, individual fundraising efforts, or foundations named for individuals.
