Easy to Use
Just download and complete the Employer Secure Access Request form, and return it to CDPHP to register. Your company’s health benefits administrator or authorized staff member will then be able to log into the secure site to complete tasks securely online.

The secure site is available to most employer groups with commercial a CDPHP, CDPHP UBI, or CDPHN plan. (It is not designed for chambers, associations, and government programs enrollees.)
