As a base benefit with most EPO, PPO, HDEPO, and HDPPO plans, there’s no additional cost for your company.
HMO Travel Rider
A travel rider is also available for purchase with HMO plans. This option is ideal for those with employees who travel often or reside in other states for part of the year. National coverage is available with a $3,000 annual benefit maximum.
