CDPHP offers Medicare-eligible beneficiaries a variety of Medicare plans for 2009. You will find all the specific information you need about each CDPHP Medicare Choices plan in the documents below. We offer medical-only plans as well as medical plans with Part D prescription drug coverage.

CDPHP Medicare Choices HMO Plans are available to eligible individuals who have both Medicare Parts A and B and reside in Albany, Dutchess, Montgomery, Rensselaer, Saratoga, Schenectady, Schoharie, or Ulster counties of New York state.

CDPHP Medicare Choices PPO Plans are available to eligible individuals who have both Medicare Parts A and B and reside in Broome, Chenango, Columbia, Delaware, Dutchess, Fulton, Greene, Hamilton, Herkimer, Madison, Montgonery, Oneida, Otsego, Tioga, or Ulster counties of New York state.
