CDPHP Medicare Choices HMO Plans are available to eligible individuals who have both Medicare Parts A and B and reside in Albany, Dutchess, Montgomery, Rensselaer, Saratoga, Schenectady, Schoharie, or Ulster counties of New York state.

CDPHP Medicare Choices PPO Plans are available to eligible individuals who have both Medicare Parts A and B and reside in Albany, Broome, Chenango, Columbia, Delaware, Dutchess, Essex, Fulton, Greene, Hamilton, Herkimer, Madison, Montgonery, Oneida, Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, Tioga, Ulster, Warren, or Washington counties of New York state.
