If you are an asthma or COPD sufferer, CDPHP can help you learn how to manage your symptoms and feel better. This service is free and available to CDPHP members only.

Here are some examples of what may be covered in a one-on-one session:

• Information and tools that can be sent to your home.
• Your medications and their side effects.
• Preparing for your doctor visit.
• An asthma or COPD action plan.
