Formulary Changes

  • Effective July 1, CDPHP will remove all brand name proton pump inhibitors (PPI’s) and implement a step-therapy protocol for the generics in this class. In addition, a new step therapy protocol in the HMG-CoA reductase inhibitors (statins) class requires use of generics first. All brand name statins will be removed from the formulary except Lipitor 80mg (Tier 2) and Crestor 40mg (Tier 3) that a member may receive after the step therapy protocol is met. Please discuss your current medication(s) with your physician(s) to determine the most clinically and cost-effective options available.

CDPHP and Partners Offer Screenings

  • CDPHP, Bellevue Woman’s Care Center, and Price Chopper are again launching a mammography campaign aimed at providing screenings in our area’s urban and rural communities. Check for locations near you.
