CDPHP is pleased to be affiliated with Caremark® to help us manage our members’ pharmacy benefits. This partnership gives you convenient online access to detailed, reliable pharmacy benefits information.*
Once you’ve registered at Caremark.com, whenever you log onto the secure site, you can see your plan-specific information, such as:
- Whether a particular drug is covered by CDPHP.
- How much you can expect to pay for the drug.
- Generic and therapeutic alternatives for the drug.
Register at Caremark.com
The first time you use the Caremark.com site, you will need to register. Click "Not Registered?" As prompted, fill in the required fields. In the field labeled "Prescription Card ID," type in the first nine digits of your CDPHP member ID number.
Visitor Formulary Search
Visitors** are welcome to access basic details on the CDPHP drug formulary, including coverage information. This feature is set up using a common three-tier drug plan ($10/$25/$40) for illustration purposes only. Please note that you must proceed through all steps of pricing in order to receive complete coverage information.

**If members use the visitor formulary search link, you will not see your plan's specific drug coverage and benefits. To see information tailored to your plan, please use the member link above and register at Caremark.com.
Note: If you have used the visitor formulary link, you will need to close all browser windows and return to the CDPHP Rx Corner before you can log in as a member. Otherwise, you won’t be able to view your plan-specific information on the Caremark Web site.
