An exclusive provider organization (EPO) provides extensive benefits with the freedom to see any physician in the CDPHP Universal Benefits, Inc. (CDPHP UBI) network without a referral.

Convenient, Cost-effective

• No in-network claim forms or referrals.
• No charge for certain preventive care, including routine annual physicals, immunizations, mammograms, Pap smears, and prostate cancer screenings.
• Worldwide coverage for emergency care.

Groups may also add riders to cover other benefits such as drug coverage and extended student coverage.

Life Points^SM

• Now available with more plans, Life Points^SM rewards members with up to $365 worth of points for completing healthy activities, such as joining a gym, attending an annual physical, or buying healthy foods.
