A High-Deductible EPO or PPO provides employees with the freedom to see providers within the CDPHP Universal Benefits, Inc. (CDPHP UBI) network without a referral. Members with the HDPPO may also visit non-participating providers at an out-of-network benefits level. The higher deductibles mean more affordable premiums.
It’s All About Options
- Once the deductible is met, services may be covered in full or a coinsurance may be required.
- No primary care physician or referrals required.
- No charge for many preventive health care services including annual physicals, Pap smears, and mammograms provided in-network.
- Access to a national network of more than half a million providers. If an employee with an HDPPO plan chooses to see a non-participating provider, benefits are provided at the out-of-network level, with higher out-of-pocket costs.
- Worldwide coverage for emergency care.

Employees may also have the option to open a Health Savings Account (HSA), enabling them to take an active role in managing their own health care costs, by paying for qualified health care expenses using tax-advantaged* dollars. Learn more about HSAs at the U.S. Treasury Web site..
CDPHP has established a referral relationship with HSA Bank™. HSA Bank will assist with account set-up and expense tracking and offers a variety of ways to invest HSA dollars.

Life PointsSM
- Now available with more plans, Life PointsSM rewards members with up to $365 worth of points for completing healthy activities, such as joining a gym, attending an annual physical, or buying healthy foods.
