Beyond the Network

A preferred provider organization (PPO) gives employees the freedom to visit CDPHP Universal Benefits, Inc. (CDPHP UBI) providers without a referral and see non-participating providers if they choose.

The Freedom to Choose

• Employees can see a doctor inside the network and pay lower copayments or coinsurance. If they go outside the network, they pay a higher coinsurance after meeting the deductible.
• No primary care physician or referrals required.
• National network with access to more than half a million providers.
• No charge for well-child care and many other routine preventive health care services, including annual physicals, Pap smears, mammograms, and prostate cancer screening provided in-network.
• Worldwide coverage for emergency care.

Groups may also add riders to cover other benefits such as drug coverage and extended student coverage.

Life Points^SM

• Now available with more plans, Life Points^SM rewards members with up to $365 worth of points for completing healthy activities, such as joining a gym, attending an annual physical, or buying healthy foods.
