Life PointsSM
- Now available with more plans, Life PointsSM rewards members with up to $365 worth of points for completing healthy activities, such as joining a gym, attending an annual physical, or buying healthy foods.
Helping Members Stay Healthy
- No cost for the following preventive health care services:
- Routine annual physicals for adults and children
- Associated laboratory tests and other ancillary services associated with annual physicals
- Annual mammograms, cytology screenings, prostate cancer screenings and most immunizations
- Allergy injections
- Visits to CDPHP-designated, office-based imaging and lab vendors are also free of charge. These "copayment-waived" services are available at dozens of network locations.
Hospital-Based Facility Services
- Hospital stays and most hospital outpatient services are the only services subject to the plan’s deductible and coinsurance.
- Once the member meets the deductible, CDPHP covers 80 percent of the hospital stay, including all ancillary services.*
- Physician services during hospitalization are covered in full but subject to the deductible.*
* With the transitional PPO plan when using the out-of-network benefits, the member is also responsible for the difference between non-participating provider charges and an allowed amount, even after the coinsurance maximum has been met.
