A High-Deductible EPO or PPO, offered by CDPHP Universal Benefits Inc. (CDPHP UBI), gives you the freedom to see providers within the CDPHP UBI network without a referral. Members with the HDPPO may also visit non-participating providers at an out-of-network benefits level.

Health Savings Accounts

The advantages of an HSA include:

  • An incentive to save for future medical expenses, using pretax dollars.
  • Earn interest tax-free.
  • Expenditures are never taxed as long as they are used for qualified medical expenses for the member or a dependent, such as: doctor visits, prescription drugs, over-the-counter medications, or premiums for long-term care insurance.
  • Contributions may be made to the HSA by the member, the individual, or employer.

Please Note: The High Deductible PPO Plan is underwritten by CDPHP Universal Benefits, Inc. (CDPHP UBI). You must comply with CDPHP UBI’s managed benefits program as set forth in the contract to receive the maximum benefits for all services. Failure to do so will result in you being responsible for an additional payment of 50% of the allowed amount up to a maximum of $500 for each service otherwise payable, in addition to the applicable deductible, and/or coinsurance.
