CDPHP is a physician-directed health plan widely known for its extraordinary customer service. Established by doctors in 1984 as an independent, not-for-profit health plan, our mission is to provide quality health care at a reasonable cost.

- Staff of capable, friendly provider service professionals, available weekdays from 8 a.m. to 5 p.m. to answer your telephonic inquiries regarding claims, eligibility verification and benefits.
- A team of provider service specialists is also available to meet with you in your office, to provide education on our plans and assist as needed.
- Physicians' Academy™ program gives participating physicians an opportunity to recognize the efforts of their peers.
- The Secure Provider Site, a user-friendly, browser-based application offers around-the-clock access to:
- Claim payment information.
- Member rosters, eligibility, and other coverage information.
- Prior authorization guidelines and resource coordination and pharmacy policies.
- Provider Office Administrative Manual.
- Claims information regarding medications, diagnostic testing, laboratory testing, and radiology services, emergency room visits, hospital admissions, and laboratory testing.
- A range of preventive care services is available to members at no charge.
