You may be eligible to participate in a nationally recognized Diabetes Prevention Program that can reduce your risk of developing type 2 diabetes – and you can get reimbursed for up to $500 for completing it, too.*

About the Diabetes Prevention Program

The Diabetes Prevention Program gives you the opportunity to meet with a trained lifestyle coach over the course of 12 months to learn ways to reduce the risk of developing diabetes, from healthy eating and physical activity to problem-solving and coping skills. The program begins with 16 weekly sessions and is followed by monthly maintenance.

Talk to your doctor about this program! You will be considered eligible for the program if you meet the following criteria:

  • ✔ Must have a pre-diabetes diagnosis or history of gestational diabetes

  • ✔ Must be at least 18 years old

  • ✔ Must have a BMI ≥ 24 or BMI ≥ 22 if Asian

  • ✔ Cannot have a previous diagnosis of type 1 or type 2 diabetes

  • ✔ Cannot be pregnant

CDPHP Reimbursement for Completion of the Program

Upon completion of the program, CDPHP members can be reimbursed up to $500 per subscriber (family), per plan year.* To request reimbursement, submit the CDPHP Diabetes Prevention Program Reimbursement Form, along with the required documentation to CDPHP.

 

*Refer to your contract to determine eligibility for the reimbursement. Eligible members will be reimbursed upon submission of the required materials, including the original, paid receipt from the program, along with a completed program form. Reimbursement for the program applies to the plan benefit year in which the member paid for the services.
