Capital District Physicians’ Health Plan, Inc., CDPHP Universal Benefits, Inc.®, and Capital District Physicians’ Healthcare Network, Inc. (“CDPHP”) provides alerts via short messaging service (“SMS”) to send you information for healthcare purposes.
When you provide CDPHP with your cell phone number, you are providing consent to receive SMS messages from CDPHP, and you agree to these Terms & Conditions.
You may opt out from receiving SMS messages at anytime.
Compatible carriers include: AT&T®, T-Mobile®, Verizon Wireless, Boost®, U.S. Cellular™, Cellular One®, Metro by T-Mobile®, ACS/Alaska, Cellular One® of East Central Illinois, Centennial Wireless, Cox Communications®, EKN/Appalachian Wireless, GCI, Illinois Valley Cellular, Immix/Keystone Wireless, Inland Cellular, Nex-Tech® Wireless, Thumb Cellular, United Wireless, Cellcom®, C Spire, Cricket®, altafiber, and Virgin Mobile.
CDPHP, in its sole discretion, may change or discontinue SMS messages at any time, without prior notice to you.

SMS MESSAGING IS NOT SECURED. CDPHP IS NOT LIABILE FOR ANY OF THE FOLLOWING RELATED TO SMS MESSAGING: DELAY OR FAILURE OF DELIVERY OF SMS MESSAGES TO OR FROM ANY PARTY; TELECOMMUNICATIONS ISSUES OR FAILURES; DISSEMINATION OF INFORMATION WITHIN A SMS MESSAGES TO A PHONE NUMBER THAT NO LONGER BELONGS TO YOU IN THE EVENT OF YOUR FAILURE TO NOTIFY CDPHP IN WRITING OF A PHONE NUMBER CHANGE; INTERCEPTION OR VISIBILITY OF SMS MESSAGES BY A THIRD PARTY (INCLUDING YOUR MOBILE, CELLULAR OR SATELLITE CARRIER); TAMPERING WITH THE INTEGRITY OR VALIDITY OF SMS MESSAGES BY A THIRD PARTY; ANY LOSSES OR INJURIES OF ANY KIND RESULTING DIRECTLY OR INDIRECTLY FROM A SMS MESSAGE; OR INACCURATE OR INCOMPLETE CONTENT WITHIN A SMS MESSAGE.
