LATHAM, NY – June 18, 2024 – Community Care Physicians PLLC (CCP) and CDPHP today officially opened the doors to their 263,000 square foot, state-of-the-art medical arts complex, aptly named Wellness Way. The new facility is also the new headquarters for CCP and CDPHP.
Wellness Way is a profound symbol of the next step in the integration of services between CCP and CDPHP. For patients, Wellness Way offers a seamless experience, allowing them to see their providers, have blood work or imaging done, fill prescriptions at ConnectRx pharmacy, and get answers to insurance questions or enroll in a health plan at CDPHP Customer Connect.
