About the Landmark Health Program
Based in Latham, Landmark Health employs a team of health care specialists who provide in-home personalized care and 24/7 support to meet your specific health needs at no cost to you. These specialists include: doctors, nurse practitioners, nurse care managers, social workers, pharmacists, dietitians, and behavioral health practitioners.
To be eligible for Landmark programs, members must be enrolled in one of our Medicare Advantage plans in Albany, Columbia, Rensselaer, Saratoga, Schenectady, Warren, and Washington counties and meet certain health criteria.

How do I get started?
CDPHP has partnered with Landmark Health to provide you with personalized care that focuses on you and your specific health needs, with no cost-share for you. The purpose of this program is to keep you feeling healthy, out of the hospital, and in charge of your health.
How much will the program cost me?
This program is available with no cost-share for you.
I already have a doctor. Can the Landmark Health program still help me?
Yes! This program does not take the place of the care you receive from your doctor. Your personal Landmark Health Provider and Care Team will work with your doctor to ensure that they are kept up-to-date and provide support for effectively managing your health, medications, and information.
I do not trust anyone coming to my house. What steps will be taken to put my mind at ease?
Please be assured that all Landmark Health Providers are board-certified doctors or nurse practitioners.
In addition, the Landmark team follows a standard protocol before your scheduled visit to make you feel comfortable. Prior to your first visit, your Landmark Health Provider will contact you and introduce him/herself. All Landmark Health providers will have Landmark Health ID.
What does a visit from Landmark Health include?
Each interaction from your Landmark Health Provider and Care Team is personalized to your health needs. It includes services much like those you would receive in an office visit with your PCP, but in the comfort of your home.
Am I required to participate in the program?
If you qualify, that means CDPHP has identified you as potentially benefiting from these types of services. You are not required to participate in this program, but if you have complex health needs, this program may be able to help you stay healthy and out of the hospital.
What does Landmark Health do with the information they collect?
Your Landmark Health Provider and Care Team will use the information they collect to assess your individual needs and develop a plan for working with your doctor to deliver the best care possible.
Is this program available over the phone?
You’ll have direct phone access to the Landmark Health Care Team 24 hours a day, seven days a week by phone. Please note that the providers need to do a physical assessment and get to know you through face-to-face encounters in order for the program to fully meet your health care needs.
Is this going to impact my current health plan?
This program in no way impacts or changes your current health plan.
